TMJ Dysfunction

Earlier this year, I completed a TMJ (temporomandibular joint) physiotherapy course focused on the assessment and treatment of jaw-related conditions and wanted to share some reflections on the experience and key clinical takeaways. It was a very practical course that reinforced how common jaw dysfunction is, and how effective physiotherapy can be in managing TMJ dysfunction.

One of the main messages from the course was just how often jaw-related problems contribute to everyday symptoms that people may have been living with for a long time, such as:
• Jaw pain and stiffness
• Clicking or popping sensations in the jaw
• Difficulty or limited mouth opening
• Headaches linked with jaw tension
• Muscle tightness and fatigue in the jaw and face

These symptoms are often influenced by increased muscle activity and overload through the jaw system, along with clenching habits and daily strain on the jaw muscles. The positive takeaway is that these presentations often respond well to physiotherapy when managed with targeted treatment.

Jaw muscle overload and clenching habits

A key focus of the course was understanding jaw muscle overactivity. Many people are unaware they clench or grind their teeth (bruxism), especially during sleep or periods of stress. Over time, this can lead to:
• Morning jaw soreness or tightness
• A tired or heavy feeling in the jaw muscles
• Headaches (often around the temples)
• Discomfort or fatigue with chewing
• Teeth grinding (bruxism) during sleep or stress

Rather than being a structural issue, these symptoms are often driven by increased muscle load and overactivity. Management includes education around clenching patterns, along with hands-on manual therapy techniques to reduce muscle tone and improve function.

Hands-on treatment and internal techniques

The course highlighted the importance of manual therapy as a key part of treatment. This includes external soft tissue work to the jaw and neck, as well as internal techniques targeting the muscles of mastication. Internal treatment can be particularly effective for the masseter, temporalis and lateral pterygoid muscles, which are often difficult to access externally but play a major role in jaw pain and restriction.

These techniques are used alongside simple jaw exercises to support improved movement, reduce symptoms, and restore normal function.

The role of physiotherapy in TMJ care

Physiotherapy plays an important role in helping people manage jaw pain and related symptoms without invasive treatment. Management may include:
• Hands-on treatment to reduce muscle tension, including internal and external soft tissue techniques
• Exercises to improve jaw strength, control and range of movement
• Treatment for the neck and upper spine, which often contributes to jaw tension and overload
• Education around clenching patterns and load management

A key takeaway from the course was that outcomes are often best when manual therapy and exercise are used together as the main approach to treatment.

Final thoughts

The TMJ physiotherapy course was a valuable learning experience that highlighted how common jaw-related symptoms are, and how effectively they can respond to physiotherapy. It reinforced the importance of targeted hands-on treatment and exercise-based rehabilitation in managing TMJ dysfunction. When these approaches are applied appropriately, many people experience meaningful improvement in pain and function.
